1. What kinds of paperwork I will need to bring to my appointment?

i693 form with prefilled first 2 pages; patient registration form; history form, TB screening form, insurance, ID.

2. What happens if I don’t have a printer at home?

You can email the forms to yuwangmd@hotmail.com

3. Do you need my immunization records?

Yes, if you have one.

4. What happens if I couldn’t find my immunization records?

Please at least bring in the evidence of COVID vaccine. I can take care the rest of the vaccines.

5. Do I need to do labs?

Mostly like yes.

6. Do I need to be fasting for my labs?

No.

7. Can I give the urine sample if I am having period?

Yes.

8. Do I have my blood drawn at your office?

Yes. There is a $10 lab processing fee.

9. Will I be able to get a sealed envelope?

Yes, and you will also get a copy before I seal the envelope.

10.For age 65 and above, what kind of pneumococcal vaccine do I need?

PPSV23.

11.How much you charge for this exam?

$200. Your insurance may cover the vaccines in full and part of your labs.

12.How much you charge the labs?

I don’t charge the labs. Either Quest Diagnostics or LabCorp will send you a bill.

13.What is the processing time for labs?

It depends on the types of the lab. It can be 5-7 business days.

14.Do I need the chest x-ray?

Only if your TB blood test is positive.

15.Can I get the chest x-ray if I am pregnant?

We will discuss this at your visit.

16.Do I need to come in to pick up the form?

No. You can send someone in to pick it up for you.

17.Do I need to make an appointment to pick up the form?

No. Walk-in during my business hours will be fine.
